I have An ATi Radeon HD6850 2gb card, but of late it has started giving me a problem when trying to play games, some games Like Assassins Creed Black Flag, Call of Duty Black Ops II and others have no problems running, however Call of Duty Ghosts and now Fallout 3 are crashing when i try to play them. they will be playing ok for a time then they will freeze and crash back to desktop or just stay frozen and i will have to use task manager to close the game, the pic below is the error i get when it crashes back to desktop. is this a driver error or is my card on its way to the bin ?

help please

ok it hasnt put the jpeg up so here is what it says.
