I bet your computer is fine since it is that new.
It's a driver issue since 12.3 and i think 12.8 were last drivers that worked somewhat good.
There are really hundreds of thousands of users having the same issue and most of the tech nerds are suggesting people to check their RAM, PSU, Windows if there are things to mess up your card/cards, because NO it isn't driver issue never. Don't make me laugh so hard. 
AMD messed up drivers at some point so just roll back to 12.8 or earlier version which works for best and if rollback doesn't help you anymore you should uninstall and remove all and anything related to AMD/ATI you can find in registry and all folders plus uninstall your card from device manager and check "delete the driver software for this device", boot and delete anything you can find in safe mode, boot and install 12.3 or 12.8 drivers and do not update drivers before it is fixed.
Removing TDR or adding more delay doesn't fix that.
