AMD Catalyst Driver Version, and Driver History

about to be the latest beta just to see if it fixes my issues.

Operating System
-windows 7 ultimate 64bit sp1

Issue Details
my computer will not stop having seizures, so to speak, on the left monitor, the picture and programs on it act as if they are fine behind the screen in windows itself, but the screen starts having a seizure and the screen then shows nothing but bars and lines, of which the only way to get rid of them is to either put the pc to sleep, or into hibernation and wake it right back up.  the other issue is that the ati driver crashes, and attempts to recover 3 times, failing every time, and then proceeds to cause windows to blue screen.

dual dvi displays, both on their own dvi cable, no splitters.

Applications and Games

it does not matter what I am doing, because no mater what I do, browse the web, watch youtube, play a game, my left monitor always starts flickering screen wise, kinda like tv static, but instead of dots its lines and bars, which the only way to get it to go away is to put the computer into sleep or hibernation and wake it back up.
