Did you completely remove all traces of the previous graphics card? If not, remove the card and run DDU choosing the appropriate drivers to uninstall. Reboot. Download to your desktop the drivers for your new card. Install the new graphics card and install the drivers.

Run another good registry/ windows cleaner like ccleaner or tuneup utilities and try ddu again in safe mode.

Make sure you install the latest supported driver:

http://support.amd.com/en-us/download

Reinstall os if all fail and try again.

Take a look at your PSU voltages in BIOS or with a program (hwinfo64).

Try changing pcie slots (do some careful dust cleaning first).

Clean reinstall Windows and try installing latest drivers again.

There is a high chance your registry has corruption issues, which can only (usually) be solved by formating windows.

If problem persists by then, you have to start considering it's a faulty card and testing another gpu to see if everything is ok with your other pc components (like the mobo or PSU).

Try previous/ older drivers as well.

Or just reinstall the os and try on a clean environment.

Yes, try clean installing every previous driver you can.

If they all fail, then either the card could be faulty or the PSU.

Install aida64 program and see if the psu voltages are good on idle and while playing.
