My monitor will suddenly go into a spasam of pixelated boxes, usually pink and green that sometimes blink like crazy or is just there permenetanly. My computer will then proceed to crash to blue screen and restart. Sometimes, it'll freeze, then recover itself and say at the bottom of my screen, "AMD driver has stopped responding and has recovered successfully."

I have cleaned out my computer of all dust, so it should not be a over heating problem, unless my fans are broken. But I do not hear loud humming or anything when it crashes.

This seems to have happened more frequently after I have updated my driver. I am currently using:

AMD VISION Engine Control Center 11.12, driver version 8.01.01.1215.
ATI Radeon HD 4800 Series video card (4850)
and I am on Windows 7, 64-bit

Is this a video card problem? Software issue? Or is it coupled with a hardware issue? Nothing looked broken in my PC when I opened it up.
Should I uninstall all video drivers and just install from the CD my video card came with in box?

My computer was bought and built and NCIX. Not one of their NCIX PCs, just chose out parts and they put it together for me. Got it Oct 16/2010.

I am pretty new at this computer stuff, so there may be some things I don't know about my computer yet. I'll try my best to fill this out and add update it as time goes on.

RAM ( Brand / Model / Tell us the number of sticks and if you´re using different brands): Corsair CMV4GX3M2A1333C9 4GB DDR3. 2x2GB DDR3-1333 CL9 Dual Channel Memory Kit
Video Card ( Brand / Model / Is it overclocked, and if so, to what speeds and using what utility ): Powercolor Radeon HD 4850. Don't think it's over clocked/not sure how to check.
PSU ( IMPORTANT !!! Brand & Model / Link to webpage with specs / manufacturer's website or picture of unit label ): 
Monitor ( Brand / Model / Connection used): Sony SDM-HS73. Was using two, now showing desktop only on one. Using VGA connection to video card.

Number of fans present: I remember seeing 4. One on my video card, one on my motherboard. One for my PSU and one big one attached to the back of my case.

Video Card Drivers in Use ( and tried ): Currently using 8.920.0.0 version for my Radeon 4800 series and Catalyst Control Centre 11.12
Motherboard Drivers Used: (Don't know where to find this)
Apps/Games causing issues ( and if they've been patched ): Left 4 Dead 2, Dead Island, just regular browsing on net
All major components check 100% after thorough testing ( eg. Memory & Memtest86 ): (not done yet)
System clean of viruses / spyware etc: Did scan with Avast; clean. Running Spybot for spyware.
Other things tried: Rolling back driver and reinstalling driver for Radeon 4850 from their site. Switching to only 1 moniter. Considering uninstalling current driver and reinstalling using CD that came with my video card.

Pixelations all of your screen in 2d/3d mode = bad video card 70% of the time. The other 30% is usually a crappy power supply.

If everything is working fine while your playing with your machine and then it spasms out and has a heart attack my initial thinking would be power supply. (with it possibly having already damaged your video card) Or the card may have itself just gone bad.

So if i were you, I would take it down to a local PC hardware store and ask them to plug it up and see if it works (The ones here locally do it for free whenever I've asked) Or put it into a friends PC and see if the same issue happens.

You failed to fill in that bit of information.(about whats powering your machine PSU) Its pretty important.(as it really is the heart of your PC) Take the side panel off your PC and tell us so we know if your system is actually getting the juice it needs.(there should be stickers on the side with a brand / name / 12v power.. If it all it has is the 12v 3v 5v power information write that down and post it) Alot of prebuilt systems come with garbage PSU's that simply do not cut it.

It could be overheating due to dust INSIDE the game case. Just need a screwdriver to take a bunch out and wipe away the dust on the graphic card's heatsink.

You can confirm temperatures with afterburner.

This should drop the temperatures significantly and stop the crashing if it is the issue (which is common with the 4870x2 at least). I would also recommend getting some artic silver 5 thermal paste and applying it to the chip/heatsink

I forgot to mention about heat as well. my bad. 

Can air is your friend. You can use it to blow out the heatsink. If you dont feel comfortable enough disassembling it yourself like man mentioned.

But do check the heat.

Sorry for not keeping everyone updated. I appreciate all the help everyone is offering =]

I cleaned out the dust and I don't think its due to overheating. My PSU (i forget the actual number of W, sorry) but it gives enough power for my card.

I brought it to a friend, as you suggested and tested it out on his computer, and it resulted in the same problem. He then told me that it was my video card that had crapped out on me. So I have taken out my video card since then and haven't put it back in and there have been more errors or blue screens of death-related problems with my computer.

Conclusion: faulty video card.
Solution: new video card? either that or wait, cuz I'm currently running on no video card and my computer is still good enough to run l4d (on low settings and resolution of course)

I would wait till the end of Feb/march till Nvidia release's there cards as to drive down the 7k series.. then pick up a 7950. or 7870 (depending on price budget ect..ect)

Stop being lazy, pop the two screws which hold on the side panel on your case and look at the power supply make and model, takes 10 seconds, considering a quality 750w PSU costs around $125 and a HD 7950 will cost double that

It happened to me yesterday, coming from 3 monitors on eyefinity, to a new single monitor. the problem is resolve unistalling AMD Engine Control Center and use the windows drivers for the video card. Is not hardware issue as mentioned by other. My computer is working great now.
